Admission Information - Lifeline Hospital Panvel

Welcome to Lifeline Hospital Panvel. This Admission Guide has been created to help patients and their relatives understand the hospital procedures, required documents, rules, visiting hours, discharge process, TPA/insurance guidelines, and escalation matrix. Please read this information carefully before admission or while visiting any patient..

Documents Required

Patients or relatives must bring the following documents at the time of admission:

  • Government photo ID proof (Aadhar Card / PAN / Driving License / Passport)
  • Doctor’s reference letter or admission note
  • Previous medical reports, prescriptions & investigation files
  • Insurance / TPA card (if applicable)
  • Corporate / mediclaim authorization letter (if applicable)
  • MLC documents (for accident, assault, medico-legal cases)
  • Deposit amount as per room category
  • Patients must provide accurate personal, medical, and contact information.
  • Patients must follow the treatment plan advised by doctors and nurses.
  • Patients must maintain personal hygiene and keep their surroundings clean.
  • Patients must respect hospital staff, rules, and other patients.
  • Patients must inform the nursing staff of any sudden change in symptoms.
  • Patients must safeguard their own valuables and belongings.
  • Patients are required to pay bills and charges on time as per hospital policy.
  • Patients must follow visiting hours and visitor limits.

Room Categories & Deposit Amount

Note: Room charges start from 12 noon. If admitted at night/evening, charges count as one full day (till next day 12 noon).

General Rules & Patient Responsibilities

Dos and Don’ts:

DO's

  • Government photo ID proof (Aadhar Card / PAN / Driving License / Passport)
  • Maintain cleanliness and hygiene.
  • Speak respectfully with hospital staff.
  • Follow medical instructions strictly.
  • Ask for an estimate of the hospital bill when required.
  • Use sanitizers and hospital consumables responsibly.
  • Collect all belongings during discharge.
  • Use the escalation matrix if you need support.

DON’Ts

  • Do not record videos/photography without permission.
  • Do not self-medicate without doctor’s approval.
  • Visitors below 12 years are not allowed in IPD.
  • Visitors must not enter after visiting hours
  • Smoking, alcohol, and tobacco consumption are strictly prohibited.
  • Patients must not disturb others with loud noise or gatherings.
  • Patients must not offer tips or gifts to hospital employees.
  • Patients must not take any outside medicines without doctor approval.

  • Patients must not tamper with hospital equipment or medical devices.
  • Patients must not enter restricted areas such as ICU/OT without permission.
  • Patients must not use mobile phones in restricted zones.
  • Outside food is not allowed unless permitted for medical reasons.

ICU Visiting Hours:

  • Morning: 9:00 am – 10:00 am
  • Evening: 5:00 pm – 6:30 pm

  • As per ward/department rules
  • Only one relative allowed with the patient at a time
  • Other visitors permitted only during visiting hours
  • Children below 12 years not allowed inside IPD

Additional Notes:

  • No discharge is done on Sundays and Public Holidays.
  • Visitors must follow staff instructions at all times.

Discharge Process

Step-by-Step

TPA / Insurance Guidelines

  • All TPA/corporate patients must submit their ID card when asked
  • When insurance approval is received, security deposit is refunded by cheque.
  • Photocopies of reports (x-ray, CT, MRI) needed for insurance require extra charges:
  • ₹200 per film X-ray | ₹300 per MRI/CT
  • Room upgrade: patient must pay the difference if TPA does not approve the higher category.
  • If no approval is received, the patient must pay the full bill.
  • Any difference amount between hospital estimate & TPA approval must be paid by the patient.
  • Patient cannot leave the hospital until final approval from TPA is received.
  • Emergency cases on Sundays or after 8 pm require 30% additional charges (cashless treatment not applicable).
  • If TPA rejects the claim, the patient must pay full hospitalization costs.
  • Co-payment (if applicable) must be paid as per policy.

Additional Guidelines

Payments are not accepted via cheques.

Attendants must wash utensils and clothes only in designated areas.

Outside food restrictions apply as per ward rules

Coconut shells are not allowed inside the premises; only water can be brought.

Relatives are not allowed to eat inside patient rooms.

Only one relative can stay with ICU patients (designated hours only).
